Before you start

You will need the:

  • name and IRD number of the organisation
  • total value of shares granted to all employees
  • total value of employee contributions
  • total number of employees receiving a grant of shares.

What happens next

We will let you know that we have received your grant of shares information.

Your responsibilities

You need to complete this form annually and send it to us by 31 May after the tax year ends.
