Cost of Living Payment The Government has announced a Cost of Living Payment, which will be paid from 1 August 2022. You do not need to apply for this payment. If you are eligible, we’ll pay it into your bank account. Find out more

Before you start

You’ll need your:

  • IRD number and position title (as the ‘valid owner’)
  • organisation’s name.

You’ll also need the executive office holder’s:

  • full name
  • position
  • postal address 
  • phone number
  • IRD number.

Download and fill in the IR401

Sign your IR401 and attach your supporting documentation

The documentation should support the position of the person you’re appointing into the executive office holder role.

Send us your IR401 and supporting documentation

Make a copy of your IR401 and send it with your supporting documentation by:

What happens next

We’re able to process:

  • emailed requests within 48 hours
  • posted requests within 5 days.

We’ll send a letter confirming the appointment of your executive office holder.
